Telford and Wrekin Safeguarding Partnership
The Telford & Wrekin Safeguarding Partnership is seeking an independent reviewer to undertake the role of Chair/Author for an upcoming Domestic Abuse‑Related Death Review.
Case Summary
In January 2026, a vulnerable adult was admitted to intensive care with significant and unexplained traumatic injuries identified during hospital assessment. Hospital staff raised immediate safeguarding concerns due to several earlier attendances that month in which the individual had expressed fear of returning home and had reported being subjected to controlling and physically harmful behaviour by their partner. Previous multi‑agency safeguarding alerts and MARAC information indicated a pattern of escalating risk, including disclosures of objects being thrown, physical assaults, and repeated anxiety about personal safety. Police records showed multiple historic domestic incidents between the individual and their partner spanning several years, most recently earlier in January 2026, when care workers had also voiced concerns about possible abuse. Despite these indicators, the individual had not pursued formal complaints. Their health declined rapidly following admission, and they sadly died two days after admission. A police investigation is ongoing, and the circumstances surrounding the injuries and events leading to the individual’s death remain under active enquiry.
